This policy explains how personal information, including personal health information, is collected, used and protected by this practice, and how you can access it or raise a concern. It covers this website and the speech-language services we provide.

Who we are

This is an independent practice of a Registered Speech-Language Pathologist, providing in-home and virtual services in Ontario. Questions about privacy can be sent to ellysa.vardy@gmail.com.

This website

This website has no forms, no accounts, no analytics and no advertising or tracking cookies. Our website host may keep short-term technical logs (such as IP address and date of visit) to keep the site secure and working.

If you email us, we receive your email address and your message, and use them only to respond. Email is not a fully secure way to communicate, so please avoid sending personal health information by email unless instructed. This website and email inbox are not monitored for emergencies.

Booking appointments

Appointments are booked through Jane App, an external platform. Information you enter when booking is collected and stored by Jane App and is also covered by Jane App's privacy policy.

Information we collect as part of services

To provide speech-language services, we collect information such as:

  • contact details for the client and parent or guardian
  • relevant history, concerns and goals
  • assessment results, session notes and reports
  • billing and payment records

We collect only what is needed to provide care, and we ask for consent before collecting, using or sharing personal health information. For children, consent is usually given by a parent or guardian.

How we use and share it

We use this information to assess, plan and provide therapy, to keep required clinical records, and to issue receipts.

We share personal health information only with your consent, for example with another health care provider, a school, or your insurance company. We may also share it where permitted or required by law. Service providers that help us run the practice, such as our booking and records platform, may handle information on our behalf and must protect it.

How we protect and keep it

We use reasonable physical, technical and administrative safeguards to protect personal information. Records are kept for the length of time required for Speech-Language Pathologists in Ontario, then securely destroyed.

Your rights

You may ask to see or get a copy of your records, or ask us to correct them, by emailing ellysa.vardy@gmail.com. We will respond within the time allowed by law.

If you have a concern, please contact us first so we can try to resolve it. You may also make a complaint to the Information and Privacy Commissioner of Ontario.

We may update this policy from time to time. The date at the top of this page shows when it was last updated.
